SQL query when trying to quick create contact

On one of our Sugar instances (was v5.5.1) an issue has come up where users (all users) would click Create Contact from the Contacts subpanel in Accounts module, and instead of opening the quick create form, it just displays the following: "SELECT TM.id, TM.name FROM accounts AC INNER JOIN teams TM ON AC.team_id=TM.id WHERE AC.id='***********************************' (the asterisks are the relevant account id; this happens from all accounts).

We tried repairing everything, and then we thought if we upgraded to v5.5.4 (which we were going to do soon anyway) then it might fix itself. Alas, this was not the case! Still having the same problem.

Has anyone got any ideas about this?

It seems you have done some customization but forgot to remove echo at some stage.
