I had this very same problem my self & worked out it was because I was entering the wrong username on the Database Configuration screen (attached).
The Database Administration Name has to be root
Sugar Version - 4.5.0h
Sugar Edition - SugarSuite
OS - Win 2003 Server Std
PHP Version - 5.2.0
Database - MySQL Server 5
Web Server - Apache 2.0.59
I discovered a solution to this problem that has been haunting me for 2 days now. I have a hostmonster hosting account and in their MySQL database system, when you create a database called "crm" they rename it "jcsmithp_crm" where "jcsmithp" is your database and ftp admin username. When filling out the SugarCRM Installation form that asks for database name and database username, simply fill out the entire database name, not just everything after the underscore. This solved my problem and I moved on from there. If you are still having an issue, simply delete the database you wanted to use, create a new one, replace the config.php file in your sugarcrm directory with the original installation config.php file, apply a new user to the database you just created, and reinstall sugarcrm. This worked for me and I hope it works for you!
