Updated Sugar, Activities stopped working

We just updated Sugar to Version 6.2.3 (Build 6658). For some reason, the "Activities" subpanel no longer works. For instance, if I try to add a call, task, or meeting to a lead, nothing happens. It lets me enter all the information, but when I click "save" it just takes me back to the lead without creating what I just tried to create.

Any thoughts?

I experience similar issue here. But in my case, when I click the Add Task button, it shows no form.
