Upgrade wizard error during upgrade from 4.2.1b to 4.5.0d

The error message was:

Errors Found During Preflight Check: Show Bad Queries  

Table 'sugarcrm.emails__uw_temp' doesn't exist::
    ALTER TABLE `emails` ADD COLUMN `raw_source` text AFTER `mailbox_id`

Any help/hacks will be greatly appreciated.

Windows 2000 5.00.2195 (SP4)
SugarCRM version: 4.2.1b (patched)
Patch version: SugarSuite-Upgrade-4.2.1-to-4.5.0d.zip
Apache 2.0.52
PHP 5.1.2
MySQL 4.1.21-community-nt via TCP/IP

Edit: I upgraded to 4.2.1c (which now works fine), but still the exact same error message as described above when attempting an upgrade to 4.5.0d.

I'm also receiving an error when trying to upgrade from 4.2.1b to 4.5.0d (or attempting any other upgrade for that matter.

I can upload the upgrade file just fine and it appears in Upgrade Wizard list. I then click "Install" and am presented with the "Commit" screen and button. Upon clicking "Commit" it takes me to the Upgrade Wizard: Start screen showing me the steps. When I click "Next" however, the screen goes to the Sugar directory /index.php file and displays the top menu bars but nothing else. It never continues to the next step or shows any output.

I've also tried doing some of the other upgrades but they all result in the same thing.

Any ideas? Another thread mentioned deleting some rows in the Upgrade_History database table. I have looked into this but my table is already empty.

Any ideas are welcome!
