Mambo/Sugar Case Account drop-down

Attached are file changes needed to give you the ability to select an account when saving a case if you are a contact of multiple accounts or a contact of an account who is a parent account of one or more accounts. This allows you to save cases for a child account.

This is for Sugar 3.5.* and Mambo 4.5.*
