Would this new form be in the application itself or an a form on your website somewhere? Also, would this data dump directly into a new module you are building?

    There is two ways:
The esiest: you go under admin>studio, and then customize your form using the resource given by the tool. This way you do not have to worry about persistence mechanisms because sugar will handle it for you.
The hardest: Using tequiniques as customCode or extentions of edit.view.php. This way you have a free programming, but has no support of sugar farmework. But you can do what you want.

Is just as Jmetric says, first you need to define if the form you want can be into existing ones or a complete new module. Ifs that's the case, Studio & Module Builder gives you the necessary tools to do it in a quick and easy way, you can also enhance the new modules behavior via Logic hooks to do pretty neat stuff

The advantage from using the sugar tools is like Rafael says, Module Builder lets you design the view for every form on the system (Detail, edit, quick create, search, ad search, dashlet, etc) with is visual assistant.

I have actually been having a problem with the module builder, It seems when you create a module and want to list results inside the parent' "accounts" module it only lists name and date modified, it wont display and of your custom values. I have tried over and over to change this values to filter on correctly on the page and have failed. Anyone have any idea where to edit the modules table header values? I have tried to use the layout editor and nothing works.

I have this module in a relation with accounts, it appears as a module under the accounts module, in other words if you select and account and scroll down, this is one of the modules. the arrows highlight what I am trying to customize to my values.
